Apprenticeship Enrolment Form

When you enrol an apprentice, there are several contracts and agreements that must be completed. Please fill out the form below to enroll each of your apprentices. Hygiene Sue will collect the information and send you the completed forms to review and sign.

Health and Safety - Apprenticeship Agreement

As part of the apprenticeship agreement, you are required to ensure that your apprentice is being taught in a safe environment. Hygiene Sue requires all employers to complete a health and safety assessment every two years and submit their employers' liability insurance certificate annually. Please click on the link below to check that you are up-to-date with the health and safety requirements.

It is the employer's responsibility to maintain a safe working environment at all times, in accordance with established health and safety regulations. This assessment does not replace this responsibility.

Health and Safety Tracker

Track your Apprentices progress through their courses

As an employer, it is important for you to track your apprentices' progress through their courses. To do this, you can use the apprentice tracker. Simply click on the name of your apprentice and add their date of birth to access the tracker. The tracker will show you their progress on each module, as well as their overall progress in the apprenticeship.
